SALT LAKE CITY — Thomas Michael Hayes, 64, passed away on Feb. 29, 2012.

Hayes was born May 17, 1947 to Daniel and Margaret Bradley Hayes. He married Vicky Lujan (later divorced).

Hayes is survived by his children, Michael (Jean), Jarret (Anna) and Chantel (Rob) and 10 grandchildren.

A funeral Mass was celebrated at Saints Peter and Paul Catholic Church.
