Ann Kathleen Hernandez

SALT LAKE CITY — Ann Kathleen Hernandez (“Aleen”), passed away Dec. 15, 20014. She was 70.

Hernandez did not die unexpectedly; she knew she was going home for Christmas. She loved her family especially her grandchildren; that is what she lived for. Her laugh could bring a smile to anyone’s face. She made a friend everywhere she went.

She is survived by her children: Mark Mitchell and Belen (Orlando) Avila; eight grandchildren; 10 great-grandchildren; brother Joseph (Sophie) Humphreys; and sister Nora Mary (Carmen) Mancuso.  

She was preceded in death by her husband, Rafael Hernandez; parents Harry Patka and Maria Modesta; twin sister Modesta Eileen Patka; and children Tim Mitchell, Harry Mitchell, Rafael and Thomas Hernandez.

A funeral Mass was celebrated Dec. 20 at Saint Ann Catholic Church.  

A committal service will be held at Mt. Calvary Catholic Cemetery at a later date.
