SALT LAKE CITY — Jacob Porfirio Armijo, 16, passed away May 9, 2012.

Armijo was born on Aug. 1, 1995 in Salt Lake City to Sharon and Porfirio Armijo.

Armijo was preceded in death by a grandfather, and his great-grandmothers, Thelma Leonardi and Grandma Tapia.

Armijo is survived by his mother, Sharon Armijo; brother Nicholas; sisters Teresa, Toni and Trish; grandmother Mama Lucy; grandparents Grandma Kay and Grandpa Henry; and many aunts, uncles, cousins, nieces, nephews, godparents and friends.

A funeral Mass was celebrated at Saint Francis Xavier Catholic Church.
